• Telephone number,s
• VoIP services (Skype, Zoom, and all the similar accounts),
• Address,
• Social media profile URL (Linkedin, Facebook, Github, Twitter or any other allowed URL),
• History of education (name of institution, title, period),
• Work history (Company name, job title, period),
• Knowledge of languages,
• Projects,
• Organisations I joined or events I participated,
all for the following purpose:
A. to assess the qualifications and abilities for the job I am applying for
B. to assess qualifications and abilities for the job to be opened in the future (open applications, or future job openings C&I may have)
I confirm that I agree that the controller processes my data for the purpose to which this consent applies for a period of 12 months from the date of application in the case of the job application, or 60 months from the date of application for jobs to be opened in future (open applications) and that I am aware that the controller does not intend to pass on my personal data to third countries or international organisations.
I confirm that C&I has informed me of my rights under the Regulation on the Protection of Individuals concerning the Processing of Personal Data and the Free Movement of Such Data.
I confirm that I have been informed by C&I on the repeal of Directive 95/46/EC, in particular:
1. Right of access
(a) I can obtain confirmation as to whether personal data concerning me is being processed.
(b) If personal data is accessed and the following information is being processed, I can obtain:
i. the purpose of the processing;
ii. the categories of personal data in question;
iii. recipients or categories of recipients to whom personal data have been or will be disclosed, in particular recipients in third countries or international organisations;
iv. if possible, the intended period in which the personal data will be stored or if this is not possible, the criteria used to determine that period;
v. the existence of a right to request the controller to correct or delete personal data or to restrict the processing of personal data relating to the respondent or the right to object to such processing;
vi. the right to lodge a complaint with the supervisory authority;
vii. if personal data are not collected from me, any available information on their source;
viii. the existence of automated decision-making, which includes the profiling referred to in Article 22 (1) and (4) of the above Regulation, and, at least in those cases, meaningful information on the logic involved and the importance and foreseeable consequences of such processing for the respondent.
2. Right to data portability
(a) I have the right to receive personal data relating to me, which I gave to the controller in a structured, commonly used and machine-readable format.
(b) I have the right to transfer this data to another controller without interference by the controller data provided if the processing is based on consent or a contract or processing is carried out automatically.
(c) When exercising the right to data portability based on this point, I have the right to directly transfer data from one controller to another if it is legally justified and technically feasible.
3. Right to forget (right to erasure):
(a) I have the right to obtain from the controller the deletion of personal data relating to me without undue delay and the controller has an obligation to erase personal data without undue delay if one of the following conditions is met:
i. personal data is no longer necessary to the purposes for which it was collected or otherwise processed;
ii. I object to the processing, or there are no stronger legitimate reasons for the processing;
iii. withdraw the consent on which the processing was based and if there is no other legal basis for the processing;
iv. personal data must be deleted to comply with a legal obligation under EU law or the law of the Member State to which the controller is subject;
v. personal data have been unlawfully processed;
vi. personal data have been collected in connection with the provision of information collection company services.
4. Right to processing restriction
(a) I have the right to obtain a processing restriction from the controller if one of the following conditions is met:
i. the respondent disputes the accuracy of personal data, for a period allowing the controller to verify the accuracy of personal data;
ii. the processing is unlawful, and the respondent objects to the deletion of personal data and instead seeks to restrict their use;
iii. the controller no longer needs the personal data for processing, but the respondent requests them to set, realise or defend legal claims;
iv. the respondent has objected to the processing pending confirmation as to whether the controller's legitimate reasons exceed the respondent's reasons.
5. The right to correction
(a) I have the right without undue delay to obtain from the controller the correction of inaccurate personal data relating to me, or I have the right to supplement incomplete personal data.
6. The right to object
(a) I can, at any time object to the processing of my personal data.
(b) From the moment of receiving the complaint, the controller will no longer process my personal data, unless it proves the existence of compelling legitimate reasons for processing that go beyond the interests, rights and freedoms of respondents or set, exercise or defend legal claims.
(c) In case I am not satisfied with the controller's reaction to the complaint, I can file a complaint with the national competent authority (AZOP).

I confirm that consent withdrawal does not affect the lawfulness of processing based on consent before its withdrawal.
